The wasteland kills in a thousand ways, and not all of them are quick. Diseases in Dystopia Rising LIVE are narrative engines: conditions that reshape how characters interact with the world, with each other, and with the Infection that sustains them all. They are not random afflictions or passive penalties. Each disease tells a story about the post-Fall world, about the price of survival and the limits of the biology that makes survival possible. Some punish curiosity. Some corrupt faith. Some turn the body against itself in ways that blur the line between the living and the dead. They exist to create stakes that matter and consequences that linger.
Every disease in this system follows a five-stage progression from onset to terminal. The stages are not just mechanical thresholds; they are narrative beats, each one changing how the afflicted character behaves, what they can do, and how others perceive them. Stage 1 is often subtle enough to mistake for something else. Stage 5 is almost always fatal. The space between is where the stories happen: the desperate searches for treatment, the hard choices about resource expenditure, the slow realization that something is very wrong.
Diseases spread through three primary vectors. Environmental transmission occurs within Zones of Mechanics where contaminated terrain, radiation, or biological hazards saturate the area. NPC-carried diseases travel on the claws and teeth of Zombies, Raiders, and Critters, turning every combat encounter into a potential infection event. Item-based transmission comes from handling contaminated objects, consuming tainted substances, or interacting with infected materials. Each vector creates different gameplay dynamics and demands different precautions. No disease in the system spreads through casual proximity alone; contraction always requires a specific triggering action or exposure.
Beyond the core diseases that threaten individuals and settlements, there exist Apocalypse Grade afflictions: world-altering forces that reshape entire regions. The Omni Plague severs the bond between host and Infection, making death permanent within its miles-wide zone. The Radiant Blight overwhelms the landscape with fungal abundance, lulling everything living into peaceful surrender. The Shattered Genome fuses radiation, psionics, and necrological contamination into a cascading mutation event where every act of willpower becomes a gamble. These are not diseases to be cured. They are catastrophes to be survived, endured, or escaped. Their introduction into play is always declared in advance, always transparent, and always a deliberate choice by both staff and players to engage with the highest stakes the setting has to offer.
